Welcome to South Bend Orthopaedics. We thank you for choosing us for your orthopaedic medical needs. This packet is design to provide you with the information you need prior to your appointment, so the process is convenient and comfortable for you.

What to Expect During Your Visit
Although no two injuries are the same, each patient’s visit will include some basic steps:
- Consultation
- Examination
- Tests
- Treatment
- Recovery
First, you and your doctor will discuss your injury and an examination will be conducted. Next, tests appropriate to your injury – such as x-rays, blood work or diagnostic tests – will be performed.
Once the preliminary process is complete, your doctor can focus on your specific needs. This may include a combination of medical counseling, medication, surgery, casts, splints, exercise or therapy. Your doctor will also consult with you during your recovery.
Your First Visit with South Bend Orthopaedics

Your first contact with SBO as a new patient will be with one of our staff members. They are a group of well-trained, experienced and compassionate professionals dedicated to assisting you in any way necessary. If you are unable to make your scheduled appointment, please cancel at least 24-hours in advance. Your courtesy will allow another patient to see us during that time.
Please bring the following items with you to your appointment:
- Current Photo ID
- Current Insurance Card
- Acknowledgement Form
- All Previous X-ray, MRI, CT Scans and Report within One Year
- Written Referral, if required by your insurance policy
- Written Worker’s Compensation Authorization – if applicable
