South Bend Orthopaedics has teamed with Sharecare Health Data Services to fulfill requests for medical records. This includes all information related to your treatment with our providers, imaging such as x-rays and MRIs, and billing information.
How to Request Your Medical Records
Online: You can complete your medical records request online by filling out the embedded form below. Start by clicking on the button that says, “Ok, Let’s Go!” You will be required to provide a valid email address and government-issued ID. This is to protect your health information and verify your identity. If you are using Internet Explorer, you will need to save a jpeg copy of your ID to your computer/device. Internet Explorer is unable to connect to any webcam devices. This link is intended to be used by patients only.

Frequently Asked Questions
Who can request medical records?
Only the patient or a parent/legal guardian can sign the Authorization to Release Protected Health Information form. If you are requesting medical records on behalf of a patient or acting as the patient’s representative, please include a copy of the patient’s Durable Power of Attorney for Healthcare/Conservatorship with the Authorization Form.
How long will it take to receive my records?
Your request will be processed within seven to ten business days. If you choose to have your records mailed, please allow adequate time for the postal service to deliver your information (in addition to the seven to ten business days).
How do I talk to someone about my request or the request process?
Can I pick up my records in person?
Records can be emailed or mailed – they cannot be picked up at any of our facilities.
What if I am completing the authorization form for my FMLA or disability paperwork?
A paper authorization form must be completed and sent to the SBO representative helping you with your paperwork, not to Sharecare. Not sure who you need to talk to about your FMLA or disability paperwork? Contact SBO’s Medical Records Department at 574-247-9441 assistance.
